Head of Pediatric Dentistry Clinic 

title of professor or the degree of habilitated doctor or employment as a university professor,

significant scientific and professional achievements,

specialization in pediatric dentistry 

meeting the requirements set out in Art. 113 of the Act of July 20, 2018, Law on Higher Education and Science  (uniform text Journal of Laws of Republic of Poland of 2024, item 1571, as amended) and in § 33 sec. 2 of the PUMS Statute,

declaration that the PUMS is or will be their primary place of work

1. declaration of application

2. CV containing information about education, professional qualifications and the course of previous employment along with contact details

3. self-report informing about scientific and research, didactic and educational, organizational and professional achievements

4. information on activities in the field of education of scientific staff

5. document confirming the possession of an title of professor or an academic degree of habilitated doctor or an employment as a university professor

6. list of scientific achievements with a summary (bibliometric list) confirmed by the scientific library

7. the proposed development program of the Pediatric Dentistry Clinic

8. documents confirming obtaining a specialization in pediatric dentistry

9. declaration that the candidate meets the requirements arising from the Art. 113 of the Act of July 20, 2018. Law on Higher Education and Science

10. declaration that the candidate meets the requirements specified in § 33 sec. 2 of the PUMS Statute

11. in the case of candidates born before August 1, 1972, the declaration referred to in Art. 7 sec. 1 of the Act of 18 October 2006 on the disclosure of information on documents of state security authorities from the years 1944-1990 and the content of these documents (uniform text Journal of Laws of Republic of Poland of 2025, item 1519) or the information referred to in Art. 7 sec. 3a of this Act,

12. statement that the Poznan University of Medical Sciences is or will be the candidate's primary place of work

13. consent to the processing of personal data with the following content: "I consent to the processing of my personal data for the purposes of recruitment to entrust the function of the Head of the Pediatric Dentistry  Clinic run by the PUMS. At the same time, I declare that I give my consent voluntarily and that I have read the information clause attached in the announcement of the competition.”

14. if the candidate is not an employee of PUMS, copies of employment certificates from previous workplaces and at least two opinions confirming the candidate's qualifications
